I read the book twenty years ago. The eyeball bit is about gouging the eye slowly so that it comes out intact and still connected to the optic nerve.  That way, the victim can still see yet cannot blink or turn away from whatever you want them to see - in fact the eye us already pointed at their critch so they have to watch you mutilate their wiener.

All good stuff, wonderful family read.
